Predictive Factors of Switching to Bipolar Disorder in Early Onset Depressive Disorder Patients : 5-Year Retrospective, Preliminary Study in a University Hospital

Abstract
Objectives : The aim of the study was to determine predictive factors in switching to bipolar disorder among early onset (¡Â25) depressive disorder patients by means of retrospective reviews of medical records.

Methods : Retrospective chart review of patients who were admitted to a university hospital with a primary diagnosis of major depressive disorder from January 2005 to August 2008 was conducted. We reviewed DSM-IV diagnosis and detailed clinical information at the index admission with assessments made every year after discharge to determine predictive factors of switching to bipolar disorder in early onset depressive disorder patients.

Results : The data showed that 8.5% of the patients were early onset depression. Atypical features, psychotic features, and treatment resistant depression were predictive factors between patients who were switched diagnosis to bipolar disorder and patients who were maintained diagnosis of major depressive disorder in early onset depression patients.
Conclusion : There were predictive factors of switching diagnosis to bipolar disorder among early onset depressive disorder patients. The results of this study will enhance diagnostic correctness and treatment efficiency of early onset depression patients and also will determine diagnostic approach in treating early onset depression patients.
